About the Project

Age Concern volunteers support clients for around six weeks, following discharge from hospital. The level of support will depend upon the aims of the clients’ rehabilitation package and will form part of an holistic approach to helping regain some independence

Volunteers will provide non-medical support with predefined goals and outcomes. They will assist people in making readjustments and in enhancing their physical and social situation. This may involve volunteers encouraging clients to engage in rehabilitative exercise or social activities both in the home and outdoors

Where appropriate, volunteers may provide further support, together with other agencies, for a period of up to six months in order to assist in the clients’ ongoing rehabilitation, linking them with other services to support their continued independence.

The service will support existing services and will work with health professionals throughout the period of Age Concern's involvement


Volunteers and Staff
The service will involve the use of specially selected and trained volunteers who will work in conjunction with a project Coordinators employed by Age Concern. It is required that all volunteers and staff will have an awareness of the wider framework of the complementary services. Their training will reflect the kind of support it is anticipated they will offer.

Referral Criteria
The scheme receive referrals for people living in the Unitary Authority of Brighton, & Hove who meet the following criteria, with the exception of those individuals with acute psychiatric illness or complex learning difficulties.
  • Is due to be discharged or has very recently been discharged from hospital and is likely to benefit from an holistic approach to their care and rehabilitation.
  • When what the service has to offer has been fully explained to client and the client is in agreement.
  • Where a joint care package may help to avoid readmission to hospital or residential care and where the client has contributed towards decisions on what they want to achieve.
  • When there is a health professional involved who can be consulted if necessary.

The capacity of the service to respond will depend on good cooperation and liaison over existing clients and the availability of suitable volunteers.


Referral Procedure
Referral can be made by the Statutory Services e.g. Social Workers, Occupational Therapists, Physiotherapists, Hospital Discharge and Rehabilitation teams, Health Visitors and Advisors and District Nurses. Detailed information about the client will be required at the time of referral.

Referrals can be made to the 'Horizons' project coordinators.
