Age Concern volunteers provide support to clients following discharge from hospital, or to prevent admission to hospital. The level of support will depend upon the individual needs of each older person.
We aim to provide effective empowering support to help people work towards self selected goals, volunteers visit people in their own homes on a weekly basis.
Volunteers will provide non-medical support helping the client work towards their goals. Through social support they will assist people in making re-adjustments and enhance their physical and social situation. This may involve encouraging clients to engage in rehabilitative exercises or social activities both in the home and outdoors.
Where appropriate, we may offer continued support for a period of up to six months to help maintain the clients’ ongoing independence.
This service will work closely with other agencies and health professionals to assist the client to regain their independence.
The Horizons service can also provide signposting to other voluntary services that can help with rehabilitation.
